Terms and Conditions:

 By registering for the event you understand and agree to the following:

• I understand I have voluntarily registered myself and my child for the event and we are under no obligation to participate in the event or any activities.
• I release Kourtni King, Charles King III, and Fuel My Soul, LLC of all liability for myself and my personal belongings.
• By registering for this event you take full responsibility for your own safety and well-being and full responsibility for your child's safety and well-being.
• I understand and agree to the terms and conditions of the event payment processes. 
• I hereby affirm my child and I are in good health and able to participate in the event activities. I understand this event is not medically supervised. I will agree to consult with my physician in regards to questions about my and my child's health and ability to participate both mentally and physically.
• I agree to indemnify and hold harmless Kourtni King, Charles King III, and Fuel My Soul, LLC and their respective partners against any and all claims and expenses, including attorney fees, arising out of my participation in this event and the activities throughout the event.
• I hereby waive and release Kourtni King, Charles King III, and Fuel My Soul, LLC from any and all claims, costs, liability, and expenses for any injury, loss or damage whether known, anticipated, or unanticipated arising from my participation and my child's participation in the event.
• I understand under no conditions will this be refunded.
• By registering you understand Kourtni King reserves the right to remove you from the event at any point after registration even during the event itself if necessary.
• I give permission to Fuel My Soul, LLC and Kourtni King to use photography and videography taken during the event for marketing and promotional material
• The information provided in or through this experience is for educational and informational purposes only and solely as a self-help tool for your own use
• Kourtni King and Fuel My Soul, LLC is not offering medical, mental health, or religious advice. There is no health care, medical, or nutritional therapy services provided. There is no attempt to diagnose, treat, prevent, or cure any physical, mental, or emotional issue, disease, or condition. Always seek the medical advice of your own Medical Provider or Mental Health Provider.
• The information offered during the event has not been evaluated by the Food and Drug Administration.
• You acknowledge you are participating voluntarily at this event and that you are solely and personally responsible for your choices, actions, and results, now and in the future. You accept full responsibility for the consequences of your and your child's participation with this event.
• You agree to use your own judgement and diligence before implementing any idea, suggestion, recommendation, or activity from the event.
• There are no guarantees with this experience. Success depends on your own effort, motivation, commitment and follow through. No particular result is being predicted or guaranteed from attending this event. You fully agree there are no guarantees as to the specific outcome or results you can expect from using the information you receive and activities you engage in during the event.
• There will be no legal or financial advice offered during this event. Kourtni King and Fuel My Soul, LLC are not responsible for errors or omissions of any kind. You agree to release Kourtni King and Fuel My Soul, LLC from any liability whatsoever of any loss or damage you may incur. Always seek financial and/or legal counsel relating to your specific circumstances as needed for any and all questions and concerns you now have or in the future.

• I understand activities and itinerary may change at any time.
• Activities Release Form: Breathwork, Hypnosis, and all other activities
• The Undersigned Client acknowledges that he or she has been informed of the following information and agrees to release any and all liabilities of Kourtni King, Charles King III, and Fuel My Soul, LLC:
• Services to be provided do not include the practice of medicine, as Kourtni King and Charles King, are not licensed physicians. These services are non-diagnostic. There are no promises or guarantees for results. By participating in sessions you agree to take full responsibility for the outcome.
• I understand my sessions are for the purpose of learning and my progress comes from my participation. I understand these are not medical or psychotherapy sessions. It is recommended you consult with your doctor if you have questions in regard to your health.
• I understand sessions are to enable me to better understand myself, my relationships, and make desired changes. The hypnotherapist cannot and will not predict outcomes or make decisions for me. I assume full responsibility for my actions and inactions which relate to any sessions.
• I freely and voluntarily consent to undergoing sessions conducted by Charles King III and Kourtni King. I understand that while hypnotherapy is a tool for positive change, a rare client may experience negative reactions. I accept any and all risks for any adverse reaction I may have. I forever release Kourtni King, Charles King III, and Fuel My Soul, LLC from any and all claims for liability and damages of any kind whatsoever which I may at any time have, including without limitation personal injury, emotional distress, and negligence (excluding only willful misconduct). I expressively waive all claims for indirect, consequential or exemplary damages. In no event shall I be entitled to recover more than the aggregate amount paid by me to Kourtni King, Charles King III, or Fuel My Soul, LLC. I will indemnify and hold Kourtni King and Charles King III harmless for any damages, including attorney’s fees, arising from my breach of the term of this release. I have read and understand the contents of this release, I am 18 years of age or older and I execute this release of my own free will and without undue influence.
• I have voluntarily enrolled in the breathwork activities and hypnosis sessions. I understand I am under no obligation of any kind to participate in any Breathwork, meditation, or hypnosis activity and I voluntarily enter into this waiver and release of liability.
• I understand breathwork is a personal growth experience designed to enhance the quality of life and is not a substitute for psychotherapy. I understand this breathwork activity will involve strong connected breathing and may include guided meditation. I understand breathwork can involve dramatic experiences accompanied by strong emotional and physical responses. I understand I may find breathwork physically, emotionally, and/or mentally stressful.
• I herby affirm I am in good health and able to participate in these activities. I do not have any physical or mental conditions which would impair my abilities to engage in these activities or which would otherwise endanger my health during the breathwork and hypnosis activities, or which would cause any risk of harm to myself or other participants. I understand the breathwork activity and hypnosis sessions are not medically supervised. I have hereby been advised I should talk to my physician and/or psychotherapist if I have any questions about my physical or mental ability to safely participate in these activities. If I have chosen not to obtain a physician’s consent prior to my participation, I hereby agree I am doing so solely at my own risk. I understand this is my sole responsibility to participate in activities appropriate for the current status of my health and to modify the activities to accommodate my own needs or limitations. If I have any questions or concerns about whether or not a particular activity is appropriate to my current health status, I understand it is my responsibility to ask my doctor before I participate.
• I agree to indemnify and hold harmless Kourtni King, Charles King III, and Fuel My Soul, LLC and their respective partners against any and all claims and expenses, including attorney fees, arising out of my participation in the breathwork activities, hypnosis sessions, and all other retreat activities.
• In consideration of my participation in the breathwork activities and hypnosis sessions, I hereby waive and release Kourtni King, Charles King III, and Fuel My Soul, LLC from any and all claims, costs, liability, and expenses for any injury, loss or damage whether known, anticipated or unanticipated arising from my participation in both hypnosis and breathwork.
• I acknowledge I have thoroughly read this Waiver and Release of Liability in its entirety and fully understand it. By participating, I am waiving certain rights I and/or my successors might have to bring legal action or assert a claim against Kourtni King, and/or Charles King III, and/or Fuel My Soul, LLC.
